I have exactly the same problem in Outlook 2003. When I click on send, having completed an email, sometimes the message appears as expected in italics and will then be sent correctly. Other times the message appears in normal font in the outbox & is not picked up when I click send/receive. Opening the message & clicking on send has no effect - the message remains in normal font in the outbox & will not sned whatever I try to do.
